Abstract


Gastrointestinalcomplicationsaftercardiacoperations,althoughrare,oftencarryanincreasedmortality.
Abdominalcomplicationsaftercardiacsurgeryinclude,paralyticileus,gastrointestinalhaemorrhage,gastroduodenalperforation,acuteacalcularcholecystitis,hepaticdysfunctionandischemicboweldisease,acutepancreatitisandpseudomembranouscolitis.
Althoughovertinjuryoftheintra-abdominalorgansaftercardiacsurgeryisrelativelyuncommon,whenitoccurs,itisassociatedwithahighmortalityandthusaccountsforasignificantandperhapsincreasingfractionofpostoperativedeaths.
Despitethedeteriorationoftheriskprofileofpatientspresentingforcardiacsurgery,theoverallincidenceofgastrointestinalcomplicationshasimprovedoverthelasttwodecades,whichmaybearesultofimprovementsincardiacsurgicaltechniques,anaesthesiaandintensivecare.
Acommonthemeamongthesecomplicationsisthedifficultyinmakinganearlydiagnosis.Themajorityofthesepatientsaremechanicallyventilated,requiringsedationandoftenreceivingpharmacologiccardiovascularsupport.Becauseofthesefactors,physicalexamisfrequentlyunreliableandsomeofthediagnostictools,suchasCTscans,aredifficulttoobtainduetotheinstabilityofthepatient.
