Abstract
Adjuvant chemotherapy in the management of advanced cancer bladder was hopeful expectation encouraged by the published promizing results in previous studies. Previous authors reported delay in time to progression in chemotherapy arm specially in patients without or with
• minimal lymph node involvement.
. Our study is a prospective nonrandomized phase III study in which patients with advanced transitional cell carcinoma of the bladder subjected to cystectomy and lymphadenectomy and proved to be at high risk of recurrence (PT3t/PT4., N 1 -N11, M0). These patients were alocated into two groups. (fifteen patients in each group).ln the first group polychemotherapy (M-VEC) was given after cystectomy and lymphadenectomy while the second group were subjected to cystectomy and lymphadenectomy alone.
The progression free survival and the total survival was delayed in the MVEC group than the control group specially in patient without lymphatic deposit or with minimal lymph node deJJ_osit Nl but the prognoses. was worse in patients with massive lymphatic deposit (Nil) .
This type of treatment is recommended in patients subjected to radical cystectomy and proved to be at high Tisk for recurrence and the performance status, is suitable to offer this type of treatment, aiming at delaying the progression free survival and total survival of these patients.
