Chordal preservation in Rheuma'tic patients during Mitral valve replacement, Partial versus Complete

Abstract


Left ventricular function often deteriorates after conventional MVR in patients having chronic severe MR. It has been postulated that disruption of the mitral valve apparatus at operation is a major mechanism of postoperative dysfunction. The aim of this work is to study and compare the effect of complete and partial chordal preservation on the left ventricular function in patients with chronic severe mitral regurgitation undergoing mitral valve replacement.
In this study, preoperative, and postoperative left ventricular function was evaluated in 76 patients with chronic severe MR. 44 patients underwent MVR with posterior leaflet preservation (group I) and 32 patients underwent MVR with total chordal preservation (group II).
The left ventricular function was evaluated utilizing echocardiography to measure dimensions, voliunes, and ejection fraction %pre and postoperatively.
Preoperatively, the 2 groups were comparable as regard clinical diagnosis, age, sex, body surface area and echocardiographic data.
