Abstract


The orbitotomy (means to cut into the orbit) procedures inlcude a' number of operations for access into the various orbital soft tissue compartments.

A primary step in orbital surgery (represents any translid surgery that is carried through the septum) is understanding of the orbital anatomy with strict respect for fascial planes and the relationships with paraorbital structures.

Before a descision is made as to the need for any orbital surgery, a complete evaluation of the patient is a must. The complete evaluation includes a careful medical and ophthalmic history with full ophthalmological examination.

Modem orbital imaging techniques provide critical information on the specific location of the lesions as well as their relationship to adjacent structures.

Despite improvements in the diagnostic accuracy of ancillary tests, such as neuroradiographic imaging and orbital ultrasound, in many causes of orbital pathology histopathologic verification of the disease process is needed before definitive therapy can be planned. Biopsy may be performed through an open incision or, rarely, needle aspirate. Based on histologic diagnosis, treatment may require additional deep orbital surgery or alternative medical therapy.
