Impact of diabetes duration on the extent and severity of coronary atheroma burden in type 2 diabetic patients: evaluation by Coronary CT angiography
Khaled Mohamed Sayed Abd El Hafez;
Abstract
105 type 2 diabetic patients with symptoms suggestive of CAD without past history of CAD were prospectively recruited for assessment of coronary atheroma burden by CCTA.
Patients included were divided into two groups:
Group I: 54 patients with type 1 DM duration < 10 years.
Group II: 51 patients with type 2 DM duration ≥ 10 years.
Coronary stenosis by CCTA was scored as none (0%), non-obstructive (1–49%), or obstructive (≥50%) for each coronary artery segment. For those patients, we compared the prevalence, extent, and severity of CAD, including coronary artery calcium score (CACS), atheroma burden obstructive score (ABOS), segment involvement score (SIS), segment stenosis score (SSS), and Syntax score.
Regarding characteristics of the study population, there was a significant increase in old aged patients associated with increased duration of diabetes (P < 0.001). Also, there was a significant increase of number of smokers among those with longer duration of DM ≥ 10 years (P = 0.005). Additionally, the prevalence of hyperlipidemia was more in patients with long standing diabetes (P < 0.001).
According to CCTA finding patients with long standing diabetes showed an increase in three vessels disease or LM (P < 0.001). In contrast, the same patients showed a significant decrease in those with normal (P <0.001) or single vessel disease (P = 0.006). Prevalence of two vessels disease did not differ significantl
