PATHOGENESIS AND RECENT STRATEGIES IN TREATMENT OF CORNEAL MELTING

Abstract


Corneal melting IS one of the ocular devastating conditions that occur as a result of a variety of etiologies. Stromal ulceration is typically initiated by defective healing of
the corneal epithelium. This healing defect wou l d then result in

persistent epithelial defect and the cornea becomes inflamed. Persistent epithelial defects and chronic inflammation predispose to enzymatically mediated u l ceration of the corneal stroma. The enzymes involved in corneal melting belong to the family of matrix metalloproteinases and are released by stromal cells, epithelial cells and inflammatory cells.

occuin association with a large heterogeneous group of ocular

and systemic diseases. It is a frequent complication of chemical injury, herpetic keratitis, collagen vascular diseases, nutritional diseases and autoimmune diseases as ocu lar cicatricial pemphigoid, Stevens Johnson syndrome and Mooren's ulcer.

Conventional treatment of such defects aimed at elimination of the underlying disease mechanism as well as control of inflammation and protection of the surface. In addition, significant progress has been made with the application of polypeptide growth factors to facilitate corneal wound healing.
