Abstract


he modified stoppa approach is advantageous because it offers improved exposure of the quadrilateral surface plate & posterior column while this approach is felt to be advantageous because it offers improved exposure of the quadrilateral surface and posterior column while minimizing dissection by avoiding the use of the “middle window” of the ilioinguinal approach.
Acetabular fractures mostly anterior, operated with the modified Stoppa approach achieve better anatomic reduction as compared to ilioinguinal approach. The modified Stoppa approach is significantly superior to the ilioinguinal approach in limiting operative time, blood loss, wound drainage, and blood transfusion.
Thus, the authors recommend using the modified Stoppa approach rather than the ilioinguinal approach when anterior exposure of the acetabulum is required, especially for fractures involving the quadrilateral plate.
The percutaneous approach has the advantages of minimal soft tissue dissection, minimal blood loss, lowering infection rates and iatrogenic neurovascular injury. Further high quality randomized controlled trials are needed to verify the results.
