ADULT ATTENTION DEFICIT HYPERACTIVITY DISORDER (ADHD) IN SUBSTANCE ABUSE PATIENTS

Abstract


Attention deficit/hyperactivity disorder (ADHD) is an impairing condition affecting 3–7% of children and 3–5% of
adults. ADHD was found to be a major risk factor for the development of substance use disorders (SUD) and is associated with greater addiction severity as well as worse substance use outcome compared with substance users without ADHD.
While there is a strong relationship between ADHD and SUD, less is known about the relationship of ADHD subtypes (i.e., inattentive, combined, and hyperactive-impulsive) and SUD. The inattentive subtype is characterized by inattention, distractibility, disorganization, forgetfulness, and lethargy, while the hyperactiveimpulsive subtype is characterized by hyperactivity and/or impulsiveness (e.g., interrupting, difficulty waiting turn, fidgetiness). The combined subtype includes both subtypes. Therefore, the purpose of this study was to detect adult ADHD in a sample of substance abuse patients and reveal the relation between adult ADHD and the type of the substance used by the patients.
In this study 51 SUD patients were interveiwed at inpatient wards and outpatient clinic of institute of psychiatry in Neuro psychiatry department of Ain shams university hospitals. All participants were interveiwed using the following scales; ICD-10 symptom check list, Social Classification Scale, Addiction Severity Index, Conners Adult ADHD Rating Scales Self-Report: Long-Version (CAARS-S:L) ,Kiddie-Sads-Present and Lifetime Version (K-SADS-PL).
The study concludes in general that ADHD is present in a sample of SUD patient in 16-18% using 2 diagnostic tools, Participants with ADHD showed more drug problems, legal problems and Alcohol related problems. Of all participants it was found that the more the age of the patient the less severe inattentive symptoms or total ADHD symptoms he got, meanwhile the more alcohol related problems (need for treatment) of the patient the more severe inattentive symptoms or total ADHD symptoms he got. No differences found between ADHD group and non-ADHD group, also between ADHD subtypes in the use of specific substance.
