Salpingoscopic Assessment of Endosalpinx in Infertile Patients with Suspected Fallopian Tube Diseas

Abstract
Tuboperitoneal pathology is considered now one of the main etiological • factors, accounting for approximately 40% of the causes of female infertility and has persistently challenged the infertility surgeons all-over the world (Westrom, 1980). Normal fallopian tube function is currently determined by hysterosalpingography and laparoscopy with chromopertubation (RCOG, 2004).
Salpingoscopy is the transfimbrial endoscopic direct exploration of the lumen and endosalpinx of the tube; thus refining the prediction of successful tubal function (Nezhat eta/., 2000).
This prospective cohort study was conducted m Obstetrics and
Gynecology deprutment, Assiut University Hospital, in the period between June, 2001 to July, 2004. It included 83 infertile patients having eitl1er definite distal tubal pathology (hydrosalpinx, fimbria! phimosis or peritubal adhesions) or patients having unexplained infertility.
Aim o]tlle study was to examine the accuracy of salpingoscopic score .in
predicting the reproductive outcome in infertile patients with or without distal tubal disease and to evaluate the diagnostic value of salpingoscopy in relaiion to . hysterosalpingography and laparoscopy with chromopettubation in testing anatomical and functional fallopian tube patency.
All patients were evaluated thoroughly in the outpatient clinic to allow proper inclusion of cases. Patients were subjected to hysterosalpingography, laparoscopy and salpingoscopy. Patients having distal tubal disease or pelvic adhesions were graded according to .AFS classification (1988). Patients with tubo-peritoneal disease (hydrosalpinx, fimbria! phinwsis and pelvic adhesions) were managed accordingly.
