COMPARETIVE STUDY OF BILEAFLET PROSTHETIC VALYES IN AORTIC POSITION

Abstract

The aortic valve lesion either aortic stances, regurgitation or double aortic lesion affect the left ventricle by causing hypertrophy, dilatation or both.

Also depression of the left ventricular function may occurs with increased left ventricular mass , increased EGG score, increased cardiothracic ratio, increased systolic wall stress, and a systolic pressure gradient may be present across the aortic valve.

Aortic valve replacement is usually done aiming for the left ventricle to regain its normal mass, dimension, and function, with improvement of the functional class, and the patient regains his normal activity.

This may not always be the result, as the small aortic valve prosthesis may cause an obstruction to the blood flow. Which may leave residual changes in the left ventricular mass, dimensions, or function.

Usually the left ventricular mass regresses rapidly during the first 6 months postoperatively, and may regain its normal value.

The left ventricular mass may not regress if an obstruction is present.

The left ventricular systolic function increases with the decrease of the left ventricular mass postoperatively.

The cardiothoracic ratio decreases if the heart size decrease.

The EGG score decreases with the decrease of the left ventricular dimensions, and mass, and decrease of the left ventricular work.
