Role of Diffusion-weighted MR imaging in pre and post therapeutic assessment of breast cancer.

Abstract


Breast cancer is the most prevalent and the leading cause of cancer related cancer related death among female worldwide.
Early and accurate diagnosis of the breast cancer is necessary for successful treatment and for improving the quality of life.
Several imaging modalities are used for diagnosis of breast lesions. Commonest are the mammography and ultrasonography (sonomammography). But in view of limitations of mammography and the other techniques, a considerable interest is focused on breast MRI.
MRI is becoming a powerful tool which help in detection, diagnosis and the staging of breast cancer. Based on morphology and the enhancement pattern of lesions, contrast enhanced MR providing an overall sensitivity of 90% and sensitivity of 72% in detection of breast lesions. Other indications of breast MRI including evaluation of multifocal and multicentricipsilateral disease; for contralateral site in patient newly diagnosed with cancer (especially lobular carcinoma); for evaluation of the therapy response in patient treated with treated with the neo-adjuvant chemotherapy; for evaluating patient with axillary lymph nodes or distant metastases when breast primary is not known or for axillary lymph nodes metastases with unknown primary. Additional indications include screening in patients with history of cancer treated with breast conservation therapy and solving problem of equivocal findings on the other imaging modalities.
Optimal performance of MRI breast is highly technique dependable. Patient positioning, imaging parameters including temporal and spatial resolutions, and timing of the dynamic phase imaging are of crucial importance in performance of MRI breast.
The BI-RADS system (Breast Imaging Reporting and Data System) enables consistent and standardized description of morphologic and kinetic characteristics of the breast lesions. However, many challenges are present when interpreting the breast kinetics enhancement patterns. To decrease number of the unnecessary biopsies, more precise way for differentiating between the false positive enhanced lesion and the true positive malignancies is needed.
A DW MR imaging technology has shown a great promise for detection and characterization of many tumor types, through imaging of the alterations in microscopic motion of the water molecules. DWI can provide qualitative information reflecting the cellular changes that provide a unique insights into tumor cellularity.
