The Value of Microsurgical Testicular Sperm Extraction in Functional Azoospermia

Abstract
Microdissection testicular sperm extraction (TESE), is a technique, which •.. increases sperm retrieval from men with non-obstmctive azoospermia (NOA) with minimal tissue loss.
In this work, our first aim is to verifY the value of such newly introduced approach and to evaluate its pathological consequences compared with open, classic surgical biopsy in the same patient.
Our second atm is to establish some clinical and non mvas1ve laboratory parameters to predict spenn retrieval difficulty in order to select the proper surgical technique and to organize an adequate schedule for TESE in relation to ovum pick up.
One hundred and sixty eight azoospennic patients were included in this study. According to TESE procedure the patients were divided into 2 groups (group A and B)
Group (A) n=ll6 patients underwent diagnostic TESE via taking a limited number of samples. In this group I 00 patients with bilateral identical testicular histology underwent bilateral diagnostic TESE vin the conventional method in one side and microsurgical method on the other side. The spermatozoa recovery rate by microdissection TESE was significantly higher than by conventional TESE, 47/100 (47%) versus
30/ I 00 (30%) (P < 0.05). In order to assess the-•sMety of this new procedure, sixty patients were followed up ultrasonographically for 1, 3 and 6 months.
