MULTI PARAMETRIC MRI ASSESSMENT OF THERAPEUTIC RESPONSE OF TREATMENT FOR OVARIAN MALIGNANCY

Abstract


Ovarian tumors; are the second most common gynecological tumor and is the seventh commonest tumor in women.
Preoperatively, we have to differentiate benign and malignant ovarian cancers in order to decide whether surgery is required, and which type of surgery is appropriate. Thus, it can help avoiding unnecessary surgery especially in postmenopausal women and help deciding for conservative surgery for young ones wishing to preserve childbearing potential.
TVUS is the first imaging modality that can be used for detection of adnexal mass lesion. However, lesion characterization is done with the help of MRI.
Conventional MRI provides soft tissue characterization based on different signal intensities and contrast uptake in post contrast images.
With the development of recent technologies, new functional MRI sequences are being used as dynamic contrast enhanced (DCE), diffusion weighted (DW) and susceptibility weighted (SW) imagesreflect, vascularity, cellularity, and hypoxic status, respectively.
For further qualitative and quantitative assessment of ovarian tumors the derived quantitative parameters from these techniques potentially can be used as biomarkers of prognosis and response.
In conclusion, the development of innovative, quantitative MRI techniques for potential screening and treatment monitoring applications motivates our investigation into quantitative MRI. Using exploratory data in future studies could contribute to the development of a noncontrast ovarian MRI exams using quantitative MRI techniques along with conventional image interpretation ; this would reduce scan times and cost, and could be useful for potential future studies of new screening modalities. Additionally, based on exploratory findings, future studies could evaluate the accuracy of MRI functional methods for the characterization of the ovarian masses
