emalesundergoingcesareansectionoftenwishtobeawakepostoperativelyandtoavoidexcessivemedicationsaffectinginteractionswiththeirnewborninfantandvisitors.Localanestheticsarewidelyusedtoprovidepostoperativepainrelief,butanalgesiaisrarelymaintainedformorethan4-8hourswithlargestactinglocalanesthetics(bupivacaine)afterspinalanesthesiaadministration.
Thesystemicadministrationofhighdosesofopiateshasbeenassociatedwithsideeffectsrangingfrompruritus,nausea,andvomiting,tosedationandrespiratorydepression.Subcutaneousadministrationoflocalanestheticisamethodofpostoperativepaincontrolaftercesareansection.Potentialadvantagesofsubcutaneousrouteincludenofirstpassdrugmetabolismbytheliver,improvedpatientcompliance,convenience,comfortandconsistentanalgesia.
ASpost-cesareansectionpainisanimportantissueinobstetrics.Largeamountsofopioidanalgesicdrugsareoftenrequiredinthemanagementofintensepost-operativepainwhichcausesevidentdisruptionofmother/newbornbonding.Localanestheticseitherontheirownorincombinationwithopioidsornonsteroidalanti-inflammatorydrugshavebeenemployedasanadjuncttootherpostoperativepainreliefstrategies.
