PATELLAR COMPLICATION AFTER TOTAL KNEE ARTHROPLASTY

Abstract


The patella is the largest sesamoid bone in the body, the triangular shaped patella lies anterior to the tibiofemoral joint, it is embedded in the joint capsule and attached to the quadriceps and patellar tendons. Its main blood supply is the mid patellar vessels penetrating the middle one third of the anterior surface and the polar vessels entering apex behind the patellar ligament.


The extensor apparatus of the knee slides on the lower end of the femur like a cable on a pulley. The patellar surface of the femur and the intercondylar notch form a deep vertical gutter in the depth of which slides the patella. The direction of forces and distribution of stiffness of the concellous bone of the patella and patellocfemoral contact are to be considered in T.K.A.


We must choose the proper patient and not to attempt to resurface the unrcsurfaceable patella, we must choose the proper implant system as there are so many implants, we must choose the proper method of fixation.
