Treatment Options of Inflammatory Appendicular Mass in Adults

Ronald Makmot John Denis;

Abstract

In this review regarding the conservative management of an appendicular mass, most of the authors agreed that it’s still a highly acceptable approach for appendicular mass. And that this should be followed with interval Appendicectomy in patients with persistent right lower abdominal pain according to studies done by Garba & Ahmed., 2008.
We should be careful though when considering conservation approach alone because misdiagnosis of an appendicular or colonic tumor will be dangerous to the patient. Therefore we should first confirm the diagnosis and exclude other pathologies using radiological investigations such as US, CT scan, MRI and colonoscopy according to Tannoury & Abboud., 2013.
In this review according to Meshikhes., 2011 he concluded that interval Appendicectomy can be done laparoscopically after successful conservative management without perioperative morbidity and that its increasingly adopted by many surgeons around the world.
Sakorafas et al., 2012 concluded that we can omit interval Appendicectomy after exclusion of ileocaecal pathologies other than appendicular mass
According to Bahram., 2011 early Appendicectomy for an appendicular mass during the first admission is possible, safe and cost effective and it avoids the consequences of misdiagnosis of other pathologies involving the caecum and ileum.
