Abstract


 Nephrectomy can be the last but effective treatment in some conditions of a symptomatizing small atrophic kidney. In such cases precipitating Reno vascular hypertension, or recurrent attacks of pyelonephritis and may be septicemia, nephrectomy can be considered as a lifesaving procedure.


 Nephrectomy of a small atrophic kidney can be carried out through many surgical approaches, out of them two approaches stay very suitable, regarding both advantages and disadvantages.



 Trans-peritoneal laparoscopic nephrectomy of a small atrophic kidney can be a very dependable approach, provided that, well experienced hands are available, and the theatre is well equipped to save any situation.



 Laparoscopic surgery can offer the substitute to open surgery making benefit from the advantages of laparoscopic surgery, with acceptable morbidity rates, in cases the well experienced hands are available and the operative theatre is well equipped.

 Vertical lumbotomy approach to renal surgery can be very suitable to renal operations especially children and those who had previous flank operations provided, the experienced hands are available.
