Intra – operative choledochoscopy in Biliary surgery

Abstract


The aim of our study was to evaluation the role of choledochoscopy in common bile duct exploration.

Twenty One patient had obstructive jaundice were included in this study, their ages ranged between 25 years and 75 years with a mean age of 49 years.

Each patient was subjected to thorough history and clinical examination as well as laboratory investigations (S. Bilirubin, SGOT, SGPT, Alkaline Phosphatase and S. Amylase) and abdominal U/S. CT scanning was performed for two patients. ERCP was done for twenty patients.

Exploration of the CBD was performed for all patients by the conventional method followed by choledochoscopy through supraduodenal choledochotomy. In the 18 patients who had CBD stones, choledochoscopy detected missed stones in 6 (33.3%) cases after the conventional method . After retrieval of stones closure of the CBD around aT-tube was performed for all cases. Post-operative T-tube cholangiogram was done for all these cases revealed a missed stone in one (5.5 %) case.
