Bacteriologically Confirmed Pulmonary Tuberculous Infection and Drug Resistance

Abstract
Pulmonary tuberculosis is an important respiratory infection and health problem \\Orld"1de. Early diagnosis and treatment are important components in the control of this infectious disease.
Direct smear examination of sputum is a simple and rapid diagnostic method but in patients suspected to having active pulmonary tuberculosis, in whom AFB are not demonstrated on sputwn direct smear, present a difficult diagnostic and therapeutic problem. Flexible bronchoscop)• is a useful procedure in evaluating these patients.
Antimicrobial susceptibility tests for tuberculosis take weeks and delayed therapy can lead to an increase in disease incidence.
The present study was prospectively designed and conducted on 60 patients who were admitted to the Department of Chest Diseases, Assiut University Hospital and classified into 2 groups:
1- The first group: included 30 patients with sputum smear negative suspected pulmonary tuberculosis. Fiberoptic bronchoscopy was done in all cases where bronchoalveolar lavage and bronchial biopsy were taken and examined bacteriologically and histopathologically, respectively. AFB stain of BAL was positive in 6.67% of cases. AFB culture of BAL was positive in 13.33% of cases while transbronchial biopsy was positive in 16.67% of cases.
The second group: included 30 patients who proved to have pulmonary
. tuberculosis clinically, radiologically and microscopically where sputum direct smear of every patient was positive for AFB.
