Correlation between Glycosylated Haemoglobin Level and the Severity of Premature Coronary Artery Disease in Non-Diabetic Patients

Abstract


Coronary artery disease is one of the leading causes of death worldwide, many risk factors are involved in the pathogenesis of IHD, many ways are contributing to the diagnosis of CAD; coronary angiography being the most sensitive, Many studies are evolving in this field trying to find another non invasive methods to help in an accurate diagnosis and aid in correlating severity of coronary artery disease.
In our study, we tried to demonstrate this relationship by studying the correlation between glycated haemoglobin and CHD in non diabetic patients undergoing coronary angiography. We used HbA1c level which represents the average glucose level of the preceding 2 to 3 months as a marker of glycemic status and the Gensini score as a detailed assessment of the severity of CAD.
We worked upon fifty patients; below 40 years in males and 50 years in females excluding diabetic ones going for an elective coronary angiography at Dar Al-Shefaa Hospital. History taking revealed that they complain of symptoms suggestive of CAD. Then full physical examination was performed and electrocardiography followed by 2D echocardiography were carried to them. A blood sample was collected from them and HbA1c was assessed, HbA1c levels ≥ 6.5 was excluded, then Gensini score was calculated to these patients depending on their coronary angiography findings.
We couldn't get a significant correlation between level of glycated haemoglobin and coronary artery lesions' severity, May be due to an unproved true relationship or due to reasons concerned with our study as mentioned. It's advisable to carry more studies with wide range of people distributed at distant areas and studied at multiple ages and risk factors.
