Study of relation of apelin to insulin resistance in subjects with different levels of obesity

Abstract


The number of overweight and obese subjects is taking onalarming proportions worldwide. Obesity is a multifactorialdisease that involves an alteration of energy balance.
Insulin resistance is associated with obesity and is a central componentof type 2 diabetes, leading to altered glucose and lipidmetabolism in adipose tissue, liver, and skeletal muscles.
Apelin is an adipose-tissuederived hormone that is involved incarbohydrate metabolism control. It isa circulating peptide expressed in different tissues but also produced and secreted by human adipocytes, apelin production in adipose tissue is strongly upregulated by insulin, and plasma concentrations are increased in obese and hyperinsulinemic humans.
Reliable with its putative part as an adipokine, apelin has been connected to conditions of insulin resistance.
The aim of this work was to study the relation of apelin to insulin resistance in subjects with different levels of obesity.

This study was conducted on 80 subjects who weresubdivided into 4 groups according to BMI:
• Group 1: 20 lean healthy subjects BMI 20-25.
• Group 2: 20 overweight subjects BMI 25-30.
• Group 3: 20 obese subjects BMI 30-40.
• Group 4: 20 morbid obese subjects BMI <40.

All individuals of this study were subjected to full history taking and clinical examination including (BMI, WC). Measurement of fasting blood sugar, 2hours Post glucose load, and glycosylated hemoglobin (HbA1c), fasting insulin level, Apelin-12 levels and Calculation of the Homeostasis Model Assessment of insulin resistance (HOMA-IR).
Our results denoting:

• Morbidly obese subjects havea significant higher Apelin level than obesewhohavea significant higher apelin level than overweight who havea significant higher apelin level than lean healthy subjects.

• There is a high significant statistical positive correlation between apelin and BMI, WC and HOMA-IR.

• There is no correlation between serum apelin level and HbA1C.
