re-eclampsiaisapotentiallyseriousconditionthatstillaccountsforsignificantmorbidityandmortalityforthemotherandtheneonate,complicating5-7%ofallpregnanciesandexposingthemtoa3-to25-foldincreasedriskofsevereobstetriccomplications.Although,thepathogenesisisnotfullyunderstood,itisnowwidelyacceptedthatvascularendothelialdysfunctionisthemostastonishingandtheprincipaleventinthepathophysiologyofthedisease.
Researchersinvestigatedthefactthatpreeclampsiaisassociatedwithendothelialdysfunction,ahypercoagulablestate,metabolicabnormalities,aninflammatoryresponseandatherosclerosis.Theetiologyoftheseconditionsremainselusiveandmultiplefactorsareimplicatedinpathogenesisofpre-eclampsiaamongthesemechanisms,insulinresistance.
Resistinisadipose-specificsecretedhormone,whichbelongstothefamilyofcysteine-rich,c-terminalproteins.Itisapotentregulatorofglucosehomeostasisthatisthoughttoopposetheactionofinsulininperipheraltissues;itimpairsglucoseintakebyadipocytes,increasesplasmaglucoseconcentration,andthusdecreasesinsulin.Resistinisexpressedinthehumanplacentaanditsserumlevelisenhancedinthethirdtrimesterofpregnancy;suggestingitspivotalroleinthestateofinsulinresistanceduringpregnancy.Recentstudiessuggestresistinmayplayanimportantroleinthepathogenesisofpre-eclampsiathroughtheirroleinlow-gradesystemicinflammation,atherosclerosis,andinsulinresistance.Therefore,itisreasonabletosupposethatresistinmaydirectlyorindirectlyinfluencethefunctionofplacentalendothelialcells.
