URIC ACID, CYTOKINES AND INTERCELLULAR ADHESION MOLECULE-1 IN HEART FAILURE

Abstract
Chronic heart failure is associated with hyperuricaemia and elevations in circulating markers of inflammation e.g. TNF-a., IL-6 and ICAM-1.
In this study we tried to evaluate the associated elevations of serum uric acid levels and concentrations ofTNF-a., IL-6 and ICAM-1 in patients with chronic heart failure together with the effect of hyperuricaemia control in improvement of heart failure in these patients.
The study included 40 patients with chron,ic heart failure with NYHA class IV who were admitted to Internal Medicine Department of Assiut University Hospital. All the patients received furosemide and digoxin therapy. The study also included 20 healthy persons as control group. All studied patients received allopurinol therapy 300 mg/day for one month.
The results showed that there was a high significant correlation between each of serum uric acid, TNF-a., IL-6, and ICAM-I; and chronic heart failure. The study also revealed no role of allopurinol therapy in improvement of heart failure. These results agree with some results of researches abroad and differ from others.
In conclusion, we have demonstrated that serum uric acid levels in patients with chronic heart failure are strongly related to TNF-a., IL-6 and ICAM-1.
