Uses of Spreader Graft in Septorhinoplasty

Abstract
Sheen was the first to describe the spreader graft in 1984and since that time it started to be one of the most important techniques in septorhinoplasty.
It effectively widens the dorsal septal edge and stabilizes the upper lateral cartilage to correct functional airway obstruction at the internal nasal valve and it also used for aesthetic reasons.
The evolution of open rhinoplasty technique has refined spreader graft placement and expanded indications for its use. The open technique obviously provides a clearer view of the nasal structures while making the placement and attachment of the spreader grafts easier and more precise. However, closed technique also used for its implantation.
Autogenous cartilage is the first choice to be a source of the spreader graft. In recent years, some surgeons started to use synthetic materials for creating spreader grafts.
This study was done upon 67 papers and only 13 papers were fulfilling the included criteria. Statistical analysis was done using Comprehensive Meta Analysis© version 2 (Biostat™, NJ,USA).
The results showed improvement aesthetically and functionally. So spreader graft can be used safety with very low complications by different approaches. It can be used in primary and secondary rhinoplasty as there are different sources of the grafts that can be obtained.
