Validity of three scoring systems in assessing severity and outcome in EL-Abbasia chest hospital RICU patients

Abstract


This study aimed at evaluating different scoring systems to predict the patient outcome and length of stay in the RICU.
The present study was conducted upon one hundred and thirty subjects admitted at the RICU of Abbassia Chest Hospital in the period from July 2016 till January 2017 .
All patients in this study were subjected to:
1. Detailed medical history taking,
2. Thorough Clinical examination,
3. Routine laboratory investigations & electrocardiogram (ECG)
4. Plain chest X– ray .
5. For all subjects, different severity scores (including APACHEIV, SOFA & SAPSII scores) were scored on admission to the RICU and after 48 hours.
From this study, the following were concluded:
• There was non-significant statistical difference between survivors and non survivors regarding the age, sex, smoking status, Addiction, co-morbidities (DM, hypertension, IHD, HIV, HCV) and outcome of the patients.
• There was non-significant statistical difference between survivors and non survivors regarding their diagnosis.
• APACHE IV and SAPS II score were significantly higher between non-survivors than survivors on admission and after 48 hours of admission but still couldn't predict mortality among studied cases.
• SOFA score showed non-significant statistical difference between survivors and non-survivors on admission and after 48 hours and couldn't predict mortality among patients.
• APACHE IV score on admission and after 48 hours showed negative correlation with length of stay at RICU.
• On admission and after 48 hours, only APACHE IV and SAPS II scores were significant but still poor predictors of mortality among all concluded cases.
• On admission, different scores showed non-significant difference between died and lived COPD cases. After 48 hours APACHE IV and SAPS II showed significant difference between lived and died cases but still couldn't
