Assessment of care of type II diabetic patients at family care centers in Alniaryah province ,eastern Saudi Arabia

Abstract
Across-sectional study both descriptive and analytic was conducted in the two primary health care centers in Alniaryah province Eastern Saudi Arabia, for all type 2 diabetic patients who registered and on follow up in these two governmental health care centers ,in 2013,to assess the diabetic care provided for them.
All type 2 diabetic patients in Alniaryah area, attending the two primary health care centers, during the duration of the study were included with sample size of 242 diabetic.
Data collected from records for the type 2 patient, all records have been reviewed for weight, height, BMI, last reading for HbA1C, serum lipid and blood pressure within the last three months. An interviewer- administered questionnaire asking about socio-demographic data, history of DM, aspects of control, aspects of compliance, and health had been filled for every diabetic patient.
Males accounted for about (455%) of patients and (54.5%) were females.
The mean age of the studied type 2 diabetic patients is 54.5± 12.3years of age. Most
of patient were (85.3%) Saudi. 76.9% of the patients are married. The level of
education of 37.1% of the studied diabetic patients was primary or intermediate and
25.6% were illiterate or read and write. Occupations of the studied pt were highly
represented by house wives ( 47.2 % ). 56.9% of the studied pt had a monthly income
equal to 8000 SR and above.73.3% of the studied patients have their own houses.
More than quarter of the studied patients had hypertension represent (29.7% ). Most of the studied patients (84.0%) measure blood glucose at dispensary, While( 17.7%) measure it at home by their own self. Most of the studied type 2 diabetics(88.7%) referred for fundoscopy during last year. The majority of the studied patients (93.6%), received health education about DM before.
Most of the studied patients (91.7%) use their medication according to physician's order. (88.0%) of the studied patients use their medication without supervision of the family. (76.2%) of the studied patients use oral hypoglycemic drugs, while (25.5%) of them use insulin.(21.0%) of the studied patients are smokers. (82.5%) of the studied patients don’t practicing sports. (36.6%) of the studied patients have diabetic complications. (20.5%) of them had eye complication, (12.5% )had peripheral numbness, (5.7%) had atherosclerosis, (3.8%) with diabetic foot disease, (2.6%) had renal disease, (1.7%) had cardiac disease and stroke accounts for (1.4% ).
Only (6.1%) of the studied diabetic patients had their diabetes controlled. (81.1%) of the studied patients had normal blood pressure. 65.6% of the studied patients had high level of total cholesterol level. The mean body mass index were 31.4 ± 4.2.
There is a statistically significant relation between age of patients and regular use of medications, family super vision of usage of medication, smoking, practice of sport and taking care about food as diabetic patients. Also the study found statistically significant relation between the gender of patients and smoking cigarette or shisha and practice of sport. A statistically significant relation between presence of co-morbid conditions and family supervision of usage of medication, smoking, practice of sport had been found. There was a statistically significant relation between duration of

diabetes and usage of medication according to doctor order, family supervision of usage of medication, practice of sport and care about food.
Usage of regular DM medication according to doctor order and measurement of blood glucose by own self at home had a statistically significant effect on control of diabetes. The study found that, family supervision of usage of medication, practice of sport and care about food as a diabetic had statistically significant effect on occurrence of diabetic complications in diabetic patients.
Duration of diabetes, control of diabetes, were the related factors that significantly affected occurrence of diabetic complications in diabetic patients.
