Abstract


Irritable bowel syndrome (IBS) is a functional bowel disorder manifested by chronic, recurring abdominal pain or discomfort associated with disturbed bowel habit in the absence of structural abnormalities likely to account for these symptoms.
IBS is associated with other intestinal and non intestinal symptoms such as heartburn, dyspepsia, headache, chronic fatigue and genitourinary symptoms.
A precise etiology for IBS is not recognized. However, it has been shown that IBS patients have abnormal reflexes and perception in response to gut stimuli, and the individual symptoms may depend on the specific neural pathways affected. Several pathogenic factors may play a role in these disturbances, including the following; genetic, early and environmental conditioning: cognitive/ emotional adaptation; altered response to stress; inflammatory/ post-infectious processes in the gut mucosa; and abnormalities in the composition of the gut microbiota.
In 1984, Kruis et al developed a system of symptoms, physical findings, and laboratory results, and in 1990, an international working group. developed Rome I criteria in attempt to reduce unnecessary testing and provide a uniform framework for selecting patients for diagnostic and therapeutic trials in IBS.
These criteria were revised in 1999 by the Rome II working group and again in 2006 by the Rome III working group.
A number of non bacterial pathogens have also implicated in PI-IBS ( post infectious irritable bowel syndrome), including viral and parasitic organisms.
Several parasites including E.histolytica, Giardia spp, Blastocystis hominis, and Trichinella spp have been discussed as contributing factors to the development of IBS, through the relationship is less well defined.
