Serum Hepcidin as a Predictor for Blood Transfusion in Septic Preterm Neonates

H
epcidin can serve as non-invasive iron status parameter for monitoring iron supplementation in preterm infatnts.
Studies suggested that Hepcidin can be used as a marker for neonatal sepsis.
As transfusion therapy is one of the commonly used stratiegies for treatment of anemia of sepsis so we aimed
To determine if Hepcidin could be used as a predictor for recurrent blood transfusions in septic preterm neonates and to study its relation to frequency and amount of packed RBCs transfusions. It was conducted at the NICU of the pediatric hospital & NICU of Gynecological and Obstetric hospital, Ain Shams University.
The study conducted on 7o neonates and we have classified them into three groups:
1. Septic neonates who had received frequent blood transfusion.
2. Septic neonates who had received infrequent blood transfusion
3. Control group with no sepsis and did not receive blood transfusion.
Frequent blood transfusion neonates are the preterm neonates who had received more than 2 transfusions per week.
All neonates included in this study were subjected to:
Full history taking from their mothers, full clinical examination of the babies.
Laboratory was done as CBC, CRP, and and serum Hepcidin level.
Our study showed the following results:
 In septic group, frequent blood transfusion group has higher matenal risk factors for sepsis than infrequent transfusion one.
 There was highly statistical significant increase in frequency of blood transfusion and Hepcidin level in septic groups.
 Frequent blood transfusion group received more amounts of blood than infrequent transfusion one.
 Frequent blood transfusion group has higher sepsis scores than infrequent one.
 Frequent transfusion group has longer duration of hospital stay than infrequent one.
 There was positive correlation between serum Hepcidin level and systolic blood pressure, TLC, Granulucytes, CRP,amont of blood transufion,duration of hospital stay and sepsis scores ( Rodwell and Tollner). While there was negative correlation between Hepcidin and age,weight,HG,HCT and platelets.
 There was statistical significant increase in serum
