Management of long diffuse coronary lesions by PTCA and/or stenting.Immediate & short term follow-up results.

Abstract


The aim of the present study is to evaluate the immediate and six-month follow-up for restenosis rate alter coronary intervention ( PTCA and/or stenting) of long diffuse coronary artery lesions and the various risk factors which might affect the results .

This study included 56 patients with long diffuse coronary artery lesions. There were 56 lesions which were dilated by PTCA and/or stenting . Ten lesions (17.8%) underwent PTCA, 46 lesions (82.2%) underwent
stenting with 47 stents .
Direct stenting was done in I 0 lesions (17.8%) of the 46 stented lesions
.And in the rest of the lesions 36 (64.4%) PTCA was performed prior to stenting .


The lesion length was 27.9 ± 5.9 mm (range 20-45 mm). The RVD was
2.7 ± 0.41 mm(range 1.3-3.5 mm). The nominal balloon diameter was
2.7 ± 0.4 mm(range 1.5-4 mm), while the inf1ation pressure was 9.2 ±
1.5 atm (range 6- 12 atm.)
The stent diameter was 3.1 ± 0.2 mm(range 2-3.5 mm), while the
inf1ation pressure was 12.3 ± 2.2 atm.(range 8- 14 atm.).

Angiol,>Taphic success with TIMI f1ow grade 3 was achieved in all lesions
. There was one mortality (1.7%), 5 myocardial infarctions (8.9%), 4 patients (7 .I%) had stent thrombosis, there were no emergecy CABG, only 4 patients had 4 dissections (7 .I%) of a total 56 lesions . This results were concordant with some studies and better than others ofPTCA and
/or stenting in diffusely diseased coronary arterys .
Clinical follow-up was achieved in all patients through regular out-patient
clinic visits and phone calls .
