Laryngeal Mask in Pediatrics

Abstract


The nonnal airway in pediatrics start at the mouth and external nares and tenninates at the entrance of the alveoli.

It differs from adult in: Obligate nasal breathers, narrow nares, long head in relation to bodies, large tongue, higher larynx in the neck, epiglottis is long, narrow and floppy, vocal cords have a lower anterior attachment to glottis than posteriorly, the narrowest position of the larynx is at the cricoid ring.

As the LMA is tolerated at higher level of Anesthesia, it cause less couglling , straining anc: breath holding.

The LMA consists of curved tube opening at the distal end into the lumen of elliptical mask that consists of a cuff which is inflatable through a pilot tube and a ballon. There are two vertical bars at the distal end of the tube.

LMA can be applied in children by choosing the correct size ofLMA wllich presents in seven sizes.

Anesthesia must be deep enougl1 to pennit its insertion, then the cuff is inserted into the mouth and pushed until resistance is felt, then inflate the cuff, connect to gas supply, insert bite block and tape the LMA in place.
