ROLE OF ADIPONECTIN IN WEIGHT CHANGES ASSOCIATED WITH VALPROIC ACID MONOTHERAPY

Abstract


This study aims to investigate the effect of VPA on the levels of one of the novel peptides involved in glucose regulation; adiponectin, in children with apparently idiopathic epilepsy and their relation to body weight, body mass index, fasting glucose and insulin levels (and their ratio as an indication of insulin resistance), to the serum concentration of VPA and to the characteristics of the epilepsy disorder.
The study included 30 patients with idiopathic epilepsy on valporic acid monotherapy (16 males and 14 females). They were compared to 30 age and sex matched healthy control subjects (16 males and 14 females). The mean ±SD age of patients were 8.2 ± 2.6 years (range: 4.95 and 15.61 years), with mean ±SD disease duration 47.2± 29 months. The mean± SD age of onset of the epilepsy disorder was 54.7± 35.2 months. The mean ±SD seizure frequency in the last 6 months was 14 ± 28.8 episodes and the mean ±SD seizure severity score in the last 6 months was 47.9 ± 39.2, all patients were attending the Pediatric Neurology Clinic of Ain-Shams University Hospital during the period from 1 April 2013 to 29 January 2014. While mean ±SD ages of controls were 8.3 ± 2.5 years (range: 5.34-14.17 years).

All included patients were subjected to detailed medical history (Aetiology of epilepsy, age of onset of the epilepsy disorder, seizure frequency in the last 6 months , seizure severity score in the last 6 months ,and Valporic acid drug therapy (dose valproic acid dose in milligrams per kilogram per day and Total valproic acid dose in milligrams per day were calculated.), duration of therapy and adverse effects .
Through clinical examination stress on assessment of anthropometric measures and complete general and neurological examination and routine work up including; Fasting plasma glucose level ,Fasting insulin level ,Glucose / insulin ratio ,HOMA ,Serum adiponectin assay ,and Trough valproate level.
There was no statistically significant difference between groups as regard sex, using Chi-square test, with p-value >0.05.
The mean± SD height standard deviation was significantly lower among patients (-0.29 ± 0.83) compared to controls (0.21 ± 0.75) (p=0.002). And waist to hip ratio was significantly higher among patients (0.92 ± 0.07) compared to controls (0.89 ± 0.05) (p=0.017). No significant difference was detected with other anthropometric data (p<0.05).
