IMMUNOSUPPRESSIVE DRUGS IN OPHTHALMOLOGY

Abstract
Immunosupressive agents are developed primarily for the treatment of neoplastic diseases. A better understanding of the immunopathologic basis of dif ferent ocular diseases and the unresponsiveness to conventional therapies, have made their use more widely accepted in ophthalmology.
The use of mitomycin and Five-Fluorouracil has increased the success of glaucoma surgery particularly in cases prone to failure. Similarly mitomycin is used topically to prevent pterygium recurrence following surgical excision. However the use of Five-Fluorouracil in proliferative vitreoretinopathy is still experimental.
Cyclosporine is used with some success in the treatment of a variety of eye diseases as corneal graft rejection, vernal keratoconjunctivitis, uveitis, scleritis and optic neuropathy.
Several other immunosupressive drugs are used in uveitis, collagen diseases, Mooren's ulcer and certain cases of ocular neoplasms.
The immunosuppresive agents cause multiple and serious side effects, these include: bone marrow depression , gastrointestinal disturbances, bactrial and viral in fections resulting from generalized depression of immune responses and increased risk of mulignancies especially lymphoma.
