Soluble CD14 Subtype Presepsin as a Diagnostic Marker in Central Line Associated Blood Stream Infection in Intensive Care Unit Patients

Abstract
LABSI represents the second most common DA-HAI in hospital, so it’s considered one of the hardest health and financial challenges in countries health systems.
It’s associated with high cost, prolonged hospital stay and high mortalities. So the need for rapid diagnosis and treatment is critical.
Symptoms and signs are non specific for diagnosis of CLABSI and these could occur with SIRS and any infections as localized or generalized.Blood culture is the gold standard method yet it takes long turnaround time and it has poor sensitivity 30-50% so many cases will be missed and any further delay in administrationof antibiotic will increase mortality rates.Serum markers as ESR,TLC and CRP are non specific and could be affected by other factors as bone marrow suppression or liver disease.
Presepsin is elevated early in sepsis and it’s level could be correlated with the prognosis of cases. So it was a good candidate for the present study to assess it’s role in diagnosis of CLABSI in adults patients in ICUs.
This study was conducted at Ain Shams University hospital ICUs at the period from September 2018 to March 2019. After confirming cases of CLABSI by either positive blood culture or detection of microbial DNA, Serum presepsin levels were assessedby ELISA and the mean was calculated.
