Effect of Occlusal Reduction on Post-Operative Pain in Patients with Irreversible Pulpitis and Symptomatic Apical Periodontitis Treated in a Single-Visit: A Randomized Clinical Trial. (Randomized Trial)

Abstract


This clinical study was conducted to evaluate the effect of occlusal reduction on post-operative pain in patients with symptomatic irreversible pulpitis and symptomatic apical periodontitis treated in a single-visit root canal treatment using Numerical Rating Scale (NRS) at 6, 12, 24, 48-hrs post-operatively.
The study design was a double blinded randomized clinical trial as study participants and assessor didn’t know which intervention has been received. Fifty-two patients complaining from symptomatic irreversible pulpitis with symptomatic apical periodontitis in posterior maxillary and mandibular teeth were included.
After thoroughly careful clinical and radiographic examination, patients were randomly assigned into two equal groups (n= 26) each. In the intervention group; occlusal reduction were done with absence of contact was confirmed; while in the control, contacts were left intact. Root canal treatment was performed in single-visit using Revo-S Rotary NiTi. Obturation was then done using modified single cone technique with resin sealer. Pain was assessed pre-operatively, at 6, 12, 24 and 48-hrs post-operatively.
All demographic data and NRS scores obtained from patients were analyzed. Results showed similarity between the two groups regarding age, gender distribution, tooth type, arch type, canal number and pre-operative pain. Pain intensity showed, observable decrease at 6, 12, 24, till it disappear at 48-hrs with no significant difference between both groups.
• Within the limitations of this study, it could be concluded that:
1. Pain intensity decreased by time in both groups till it disappear after 48-hrs.
2. Intact occlusal surface and reduced occlusal surface showed more or less similar effect on post-operative pain in patients with symptomatic irreversible pulpitis and apical periodontitis performed in a single-visit.
