"INTERPLEURAL AND PARAVERTEBRAL ANALGESIA IN THORACIC SURGERY "COMPARATIVE STUDY

Abstract


This work was approved by department of anaesthesiology, Assiut University Hospital. This study was conducted on fourty patients, aged between 18 - 70 yr, undergoing elective thoracic surgery through posterolateral thoracotomy. The patients were classified into two groups according to the site of postoperative analgesic injection:

Group I "paravertebral group":

Included 20 patients. Postoperative analgesia was achieved with a loading dose of 20 rnl of 0.25% bupivacaine injection into paravertebral space immediately befor chest closure, then with a bolus regimen ofO.l mg!kg of0.5% bupivacaine at hourly interval.

Group II "Interpleural group":


Included 20 patients. Postoperative analgesia was achieved with a loading dose of20 ml of0.25% bupivacaine injection into interpleural space immediately befor chest closure, followed by bolus regimen of 0.1 mg!kg of
0.5% bupivacaine at hourly interval.


All patients were premedicated with morphine 10 mg 1.m and diclofenac 100 mg suppository one hour preoperatively.

The patients were observed postoperatively in the intensive care unit for 24 hr as regards analgesia, level of sedation, the arterial blood gases, haemodynamic status and side effects such as nausea, vomiting, pruritis or urinary retension.
