ESTIMATION OF IL-1B, TNF-a, PGE2, MMP-1 AND TIMP--1 IN GINGIVAL TISSUES OF PATIENTS TREATED WITH PHENYTOIN AND CYCLOSPORIN A POSSIBLE PATHOGENIC MECHANISM

Abstract
The present study was carried out to estimate levels of IL-1 p, TNF• a and PGE2 as well as immunolocalization of MMP-1 and TIMP-1, in patients treated with Cs-A or PHT with gingival overgrowth, in
addition to patients with chronic inflammatory gingival overgrowth as compared to healthy subjects. This was carried out in an attempt to understand possible mechanisms of gingival overgrowth.
A total of 30 patients with gingival overgrowth were selected. They were divided into four groups. Group (I) comprised IO patients who were receiving Cs-A, group (2) included 10 patients who were receiving PHT and group (3) IO patients with chronic inflammatory gingival hyperplasia. In addition IO gingival biopsies were taken from patients with clinically healthy gingiva and served as control (group
4).
The study also aimed to evaluate recurrent gingival overgrowth over a period of 12 months postsurgery. Clinical data was recorded at baseline before treatment and at 3, 6, 9 and 12 months postsurgery. Also, supportive periodontal therapy was carried out for all patients at each recall visit.
Results of this study showed the following: (1) Immunological results:
Drug-induced gingival overgrowth (Cs-A or PHT) and patients with CIGH showed no significant difference in IL-la level as compared to healthy subjects. While, PGE2 level extracted from gingival tissue showed a slight increase in patients with drug-induced
