Abstract


Nursing documentation is a professional and legal responsibility of nurses. It serves for ensuring continuity of care through communication, evaluation of the quality, efficiency and effectiveness of patient care, and for providing evidence for legal, ethical, financial or quality-assurance purposes and research. Also, it provides an evidence of patient progress (Dehghan, 2013).
Nursing documentation is a vital component of safe, ethical and effective nursing practice, regardless the documentation is paper-based or electronic. This is an integral part of nursing practice and professional patient care rather than something that takes away from patient care (College of registered nurses of Nova Scotia, 2012).
Aim of the study
The present study was conducted to fulfill the following aims:
1. Assessing nurses' performance toward quality documentation for patients in ICU.
2. Suggesting guidelines for quality nursing documentation for patients in ICU.
