Ultrasonographic Measurement of Cervical Length as A Predictor of Successful Induction of Labor

Abstract


Traditionally preinduction cervical assessment is based on the digital examination of the cervix and evaluation of several variants, originally proposed by Bishop.

The advantage of the Bishop score is that it can evaluate parameters such as consistency or station that may influence the outcome.

A low Bishop score prior to labor induction has been associated with failure of induction, prolonged labor and a high rate of cesarean delivery.

Also the Bishop score does not require any special equipment; however its evaluation remains a matter of controversy depending on the differences in the clinical senses of examinations.

Transvaginal sonographic measurement of the cervical length has been reported as a method which can predict labor induction outcome.

Transvaginal ultrasonography is likely more objective than the Bishop score and has been shown to have reduced intra- and inter observer variability.

Combined both cervical length measured by TVUS and modified Bishop score has a higher specificity as a predictor of induction of labor success.

The current study was done in Ain Shams maternity Hospital and included 107 pregnant women admitted for induction of labor. The 107 post date women were tested for calculation of cervical condition according to the Bishop score and measurement of cervical length by TVUS before starting labor induction, which was done using prostaglandins analogue.(misoprostol 50 micorgram).

In our current study we used the cut-off values of a modified Bishop score >4 and a cervical length ≤ 25 mm to predict successful labor induction and we concluded that the cervical length measured by ultrasound had nearby the same sensitivity of modified Bishop score in prediction the success of induction of labor( 90.5% vs 90.5% respectively) but the cervical length measured by ultrasound showing a higher specificity in prediction the success of induction (93.5%vs 53.5%).

So, Regarding the specificity the cervical length measured by ulttrasound was better than modified Bishop score .
